By completing the application form, the organization confirms that they are committed to participating in the Thrombosis Canada Facilitated Quality Improvement Program if selected as one of the 10 program sites. Teams will be selected by Thrombosis Canada Executive Committee based on the strength of the requirements and the diversity of the setting.
Application Instructions
Please complete the online form by January 31st, 2015. Participants will be notified of a decision by February 14th, 2016.
This form should be completed by a member of the organization who is accountable for quality improvement initiatives on behalf of the program site.
